Short Notice?

Enter the city and state of the main mission. Example if the mission is occurring at the NH State Veterans Cemetery then enter Boscawen and select New Hampshire. 

If it is an Honor mission with an escort and flag line enter the location of the interment or flag line as the main location.

About the Hero/Mission:

  • We do not make reference to the deceased by the terms body or cremains, all references are to be made by the name or Hero.
  • If the individual served during a wartime era it may be listed here, you may also list their job function within the branch of service.
  • If the individual served in more than one branch of service please list them chronologically, any conflicts served in should be paired with the branch of service. 
    Example: Mr. John Doe, U.S. Army – Vietnam, U.S. Air Force

 

Example: The family of Mr. John Doe, U.S. Army – Vietnam, has requested the PGR to provide an escort from the Phanuef Funeral Home in Manchester and to stand a flag line in Mr. Doe’s Honor at the NH State Veterans Cemetery on Wednesday, 10 April 2018. 

Mr. Doe served in the U.S. Army from 1969 to 1972 as an infantrymen.

If no obituary is available or not available at the time of posting just leave the field blank. If it becomes available at a later time it can be emailed to missions@nhpatriotguard.org to be added.

Plaque Order Information

If the individuals Name and Branch of Service are the minimum required information for the plaque.

If the individual served in multiple branches of service please list all branches in order of service. If he/she served during conflict please associate that conflict with the branch of service. I.E. U.S. Army – Vietnam, U.S. Air Force

If the individual served during a conflicts era please note as such. I.E. Vietnam ERA. Anyone that served from 2001 to present is/has served during the G.W.O.T. ERA

Use the additional field for any exceptional medals or achievements. I.E. Silver Star, Purple Heart, Navy Cross, Medal of Honor … etc.
